Hello guys! In some cases, we have to deal with different types of curvatures in the same tooth, but the strategy is always the same, CORONAL PREFLARING AND API...CAL PATENCY. This is a case of a retreatment of an asymptomatic upper second left molar. After anesthesia, and isolation with clamp number 14 (Hu-Friedy, IL, USA) and medium rubber dam (Coltène Whaledent, Langenau, Germany). Amalgam restoration was removed with a long surgical carbide bur (Meisinger, CO, USA) and cavity redefined with an ultrasonic tip ET 18 (Satelec, Acteon Group, Merignac, France). Endostar E3 (Poland) 30/08 (18 mm) file was used in the coronal part to remove coronal filling material and pre-flaring with a brushing in and out motion. Then a 10, 31 mm K file (VDW, Munich, Germany) was introduced to check patency in all 4 canals and take the length with apex locator (Eighteeth, Changzhou, China). As shown in the photo MB1 & MB2 were very close at the orifice and joined in the middle mesial root. The canals were shaped with Endostar E3 Azure heat treated 20/04 and 20/06 for the MB1 & MB2 progressive curvature and DB S shaped canal. Endostar E3 20/04, 20/06, 25/06 was used in the Palatal canal. The only hand file used in this case was the 10 K file to check the patency and for scouting. IrriFlex (Produits Dentaires, Vevey, Switzerland) was used for irrigation (NaOCl 6%, EDTA 17 %, sterile water) and activation was performed with EndoActivator (Dentsply Sirona, Ballaigues, Swizerland ) for a total of 1 minute in each canal. Canals were filled with warm vertical compaction with 20/05 GP points (Coltène) and EssenSeal (Produits Dentaires). Injecting technique with Obtura II gun and a 23 G tip (Obtura Spartan Endodontics, Algonquin,IL, USA) was used for back pack. See more

-Suspected to fracture :- During Shaping of such canals instruments are more suspected to fracture . despite I got patency, glide path and preparation of mid...dle third but it happended . Tip of shaping file(20 taper 0.6) fractured(2mm) at the mesiobuccal canal, Bypass was my choice .

A wrong deep margin elevation was the possible cause of the perforation (angulated margin). 1)Canals location, ...instrumentation and obturation. 2)Perforation sealing with MTA (infracrestal). 3)Coronal restoration. Although lateral lesion was presented, but lateral canal was not filled even with carrier based obturator (Thermafil). Disinfection with Naocl 5.25% only along ...with 1 week caoh2 dressing application till symptoms and sinus tract disappearance were enough to observe complete healing after 6 months. See more

What’s your method for cleaning ACCESS CAVITY post Obturation ?

Good morning dear doctors Posting a case 26 Difficulty level 99/100 Palatal canal in close proximation with maxillary sinus (27mm) ... In first sitting after pa...tency then 20.04 it bled like any thing . Took me 1 hr to control bleeding.. Closed dressing was given and adviced for cbct ... Where it came that it bifurcated into two at apical third and that sinus lining was a bit perforated ... Took advice and left it to heal for a month ... Second sitting mm by mm with eal and radiographs proceeded with utmost care and tried to negotiate the bifurcation without damaging the sinus lining (going beyond) ... Took two hours to negotiate shape irrigate and obturate the palatal canal ... Next sitting MB1 MB2 and distal were shaped .. Highly curved .. Finally done .. Next came the POR challenge .. Extremely difficult ..gingivectomy was performed .. Saddle system used ..bulk flow followed by packable bulkfill and finally it was done Could not even take a single pic ... RETREATMENT SYMPHONY .. Patient has discomfort for many months after RCT done in 37. all symptoms were disappeared after locating the missed mesiobuccal canal... that has a separate portal of exit and removing the broken file from the distal canal that allow better cleaning for the hole root canal system which is the key factor for success.
