ENDODONTICS FOR PRIMARY TEETH To this day, there is no general consensus on when and how to provide endodontic treatment for primary teeth. The published literature varies from weak (case reports are just anecdotal) to fairly good, but the conclusions vary widely. So here are just a few clinical points where there is an abundance of supporting literature. --whenever possible, it's better to try to save an infected primary tooth rather then extract it and use a space maintain...er. --A rubber dam is essential, especially with children who may suddenly move unexpectedly. --although many still recommend using NaOCl for canal irrigation, the safer irrigant for disinfection is chlorhexidine. --be very careful measuring primary root canals with EALs and never extend any file or filling material beyond the end of the root to avoid damaging the developing permanent tooth bud. --all kinds of pastes, some which include very strong and potentially damaging chemicals, have been recommended for obturating canals in primary teeth (e.g., Sargenti paste, Oxpara paste, etc.)--these pastes should be avoided! To this day, absorbable ZOE paste is generally recommended; iodoform paste and CaOH2-pastes are recommend by a number of authors too. Gutta percha is contraindicated for primary tooth obturation. --whenever possible, perform the complete endodontic treatment in one visit. Overcoming the fear and pain that children may display is very gratifying--and when these children grow up they may remain loyal patients for the rest of your career. Stephen Cohen
